Athletic Boosters Exam Day

The Benjamin Logan Athletic Department is hosting our annual Physical Exam Day for all students who are needing a physical exam for sports, camps, work, or other activities for the upcoming 2016-2017 school year.  The Physical Exam date is Wednesday, May 25 and appointments can be made by calling or texting Kim Kossel, Athletic Booster Member, at 937-539-6900.  Appointments will be from 3:00-5:30pm and walk-ups will be accepted after 5:30pm but an appointment is suggested.  Information about this exam date can be found on the Benjamin Logan website at www.benjaminlogan.org and you can also download the OHSAA Physical Exam Form to complete the required information and obtain a parent or guardians signature prior to the exam date.  Please plan ahead and get your physical exam completed on Wednesday, May 25 at Benjamin Logan High School to be prepared to the upcoming 2016-2017 school year.

If requirements are met, the district superintendent releases the student from required compulsory school attendance.
